Gift aid declaration form

If you are a UK taxpayer, the value of your donation can increase by 25 per cent under the Gift Aid scheme at no extra cost to you! To qualify for Gift Aid, you must have paid an amount of income tax and/or capital gains tax for each tax year (6 April one year to 5 April the next) that is at least equal to the amount of tax that Action for Blind People will reclaim on your donations for that tax year.

If you pay income tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self-Assessment tax return or ask HM Revenue and Customs to adjust your tax code.
